What is Rheumatoid Arthritis?
Rheumatoid Arthritis (RA) is the archetypal systemic inflammatory autoimmune disease. In RA, the immune system mistakenly targets healthy, synovial-dense joints, resulting in chronic, destructive joint inflammation. Unlike degenerative conditions, RA is characterised by an “outside-in” pathology where the immune attack starts in the outer joint envelope.
How RA Destroys Joint Tissues
The normal synovium functions as a protective joint envelope, lubricating joints and coordinating healing. In RA, the immune system mistargets the synovium, causing it to thicken, hyper-produce fluid, and develop a destructive pannus that actively erodes adjacent bone margins.
Tenosynovitis & Stiffness
The inflammatory process also attacks the tenosynovium (tendon sheaths), a major driver of profound joint stiffness. Spontaneous, bilateral De Quervain’s tenosynovitis without a clear mechanical cause can often be an early warning sign of systemic RA.
Joint Patterns & Key Demographics
- Anatomical Distribution: Primarily targets small synovial joints of the hands and feet (MCPJs and MTPJs), along with wrists, knees, and elbows.
- Symmetry: Established RA typically presents symmetrically on both sides of the body, although early-stage RA can occasionally be unilateral.
- Demographics: The peak age of onset is between 30 and 60 years, and the condition has a strong 3:1 female-to-male ratio.

Screening & Diagnostic Tools
Early referral to Rheumatology is vital to prevent irreversible joint damage. Clinicians utilise two main clinical screening tools:
- Clinically Suspected Arthralgia (CSA) Score: Evaluates insidious onset, MCPJ/MTPJ localisation, morning stiffness >60 mins, family history, and positive squeeze tests.
- The Leiden Clinical Prediction Rule: Validated algorithm integrating demographics, joint distribution, stiffness severity, and blood markers.

Laboratory Investigations
Diagnostic blood panels analyse several key inflammatory and immunological markers:
- Anti-CCP & RF: Autoantibodies highly specific and common in RA, indicating a higher risk of rapid, aggressive joint erosion.
- Inflammatory Markers (ESR & CRP): Elevated levels indicate active systemic inflammation. About 5% of patients present as seronegative with a milder course.

Evidence-Based Rehabilitation
Active exercise and progressive strengthening do not exacerbate joint inflammation and are highly safe and therapeutic for patients with RA.
- Targeted Hand Rehab (SARAH): Progressive hand exercises improve strength and dexterity, counteracting subclinical muscular atrophy.
- Whole-Body Exercise Guidelines: High-intensity resistance training is safe, causes no adverse changes to joints, and combats osteoporotic bone loss.
Frequently Asked Questions
Smoking is a powerful driver of the citrullination of proteins, which directly accelerates the development of aggressive anti-CCP antibodies. It correlates with highly destructive joint changes and a very poor response to medical therapies. Immediate cessation is critical.
Abdominal fat tissue actively secretes pro-inflammatory cytokines, which directly aggravate systemic synovitis and make medical remission harder to achieve.
Chronic, untreated systemic inflammation in RA causes a rapid and significant loss of skeletal muscle volume, specifically targeting major muscle groups like the quadriceps and hamstrings.
