Hydrotherapy & Aquatic Rehabilitation
Science-Backed Recovery in the Water
Hydrotherapy uses the physical properties of water, specifically buoyancy, hydrostatic pressure, and thermodynamic resistance, to temporarily reduce joint loading while maintaining active movement. It serves as a clinical bridge; a desensitising tool designed to “calm things down” so you can “build things back up” on land.

Mitchell Robinson
MSK & Sports Physiotherapist
Our tailored hydrotherapy sessions are conducted to seamlessly bridge the gap between active joint flare-ups and full land-based recovery. Build tolerance and get back to what you love.
How Water Changes Pain & Joint Biomechanics
Movement in water creates alternate compression and decompression cycles on joint cartilage, promoting synovial fluid mixing and delivering essential nutrients.
Mechanical Unloading & Buoyancy
Waist-Deep Water reduces axial body weight by approximately 50%, while Chest-Deep Water reduces it by up to 75%. This allows individuals with severe load intolerance, osteoarthritis, or acute flare-ups to perform functional movements without triggering pain.
Nervous System Desensitisation
Water offers a safe environment to confront feared movements without safety bracing or sympathetic guarding. Submaximal active muscle contractions stimulate pain-inhibitory pathways at peripheral, spinal, and supraspinal levels.

1
Protocol 1: Inflammatory Arthropathies
Designed for conditions like Axial Spondyloarthritis, Psoriatic Arthritis, and RA to manage flare-ups safely.
- Clinical Goal: Maintain spinal and peripheral joint mobility, mitigate aerobic fitness loss, and manage morning stiffness.
- Setup: Chest-deep water (33-36°C warm hydrotherapy pool), 2–3 sessions per week for 30–45 minutes.
- Exercises: Aquatic Walking Matrix, water-supported Thoracic & Spinal Mobility, and Gentle Cardiovascular Conditioning.
2
Protocol 2: Severe Joint Load Intolerance
Highly effective for Knee OA, and Post-Traumatic Knee/Hip Pain.
- Clinical Goal: Restore active knee/hip flexion and extension without triggering “wind-up” sensitisation or muscle bracing.
- Setup: Transitioning from Chest-Deep to Waist-Deep over 4–6 weeks, 2–3 sessions per week for 30 minutes.
- Exercises: Phase 1 (Desensitisation in Chest-Deep), Phase 2 (Graded Loading in Waist-Deep), and Phase 3 (Land Bridge).
3
Protocol 3: Athletic Active Recovery
Accelerate recovery following high-intensity strength or plyometric training.
- Clinical Goal: Reduce delayed-onset muscle soreness (DOMS) and local tissue edema.
- Setup: Contrast Water Therapy (alternating hot/cold) or Cold Water Immersion post-training or competition.
- Exercises: Contrast Therapy cycles (1-2 mins cold, 2 mins warm) and Active Water Recovery (low-intensity pool walking, flutter kicks).
Myth-Busting: What Hydrotherapy Can & Cannot Do
Water exercise is a temporary load modifier. Land-based loading is required long-term to build tissue stiffness, bone density, and peak muscle strength.
Hydrotherapy does not alter joint alignment or “fix” structural damage. It works by modulating nervous system sensitivity and joint fluid mechanics.
Avoiding land loading long-term decreases overall capacity. The goal of hydrotherapy is to build initial tolerance so you can safely transition back to land-based activities.
Your Step-by-Step Pathway
- 1. Hydrotherapy Phase: Chest-Deep Water Exercises (Calm Sensitive Tissues).
- 2. Hybrid Phase: Waist-Deep Water + Bodyweight Land Drills.
- 3. Land Capacity Phase: Full Weight-Bearing Gym & Functional Load Building.
Practical Details: How It Works
We’ve designed our hydrotherapy onboarding to be safe, accessible, and completely stress-free.
1. Initial In-Clinic Assessment
Before hitting the water, all patients require a standard in-clinic consultation at Abound Physio. This allows us to establish your baseline mobility, ensure hydrotherapy is clinically appropriate for you, and rule out any safety contraindications.
2. The Venue & Accessibility
Sessions are conducted in the heated pool at the North Lakes Aquatic Centre. The facility is fully accessible, offering both ramp and hoist access into the water for patients with severe pain or limited mobility.
3. Guided 1-on-1 Sessions
You will meet your Physiotherapist in the foyer of the aquatic centre or . Sessions typically last 30-45 minutes in the water, where your physio will guide you through your prescribed, customised aquatic rehab program.
What to Bring
- Swimming bathers / comfortable aquatic clothing
- Towel & a dry change of clothes
- Water bottle (hydration is crucial in heated pools)
- Slip-resistant footwear for the pool deck (e.g., thongs, crocs, or aqua shoes)
Fees & Billing
Your 1-on-1 hydrotherapy appointment is billed as a standard physiotherapy consult, meaning it is claimable via Private Health Insurance.
Please note: Patients are required to pay the standard casual pool entry fee directly to the North Lakes Aquatic Centre reception upon arrival.
