Understanding Gout
Gout is a highly painful crystal arthropathy characterized by the deposition of monosodium urate (uric acid) crystals within joint tissues. It features a hyperacute onset, commonly occurring during the second half of the night (2:00 AM – 3:00 AM) due to diurnal drops in protective cortisol levels.
Symptoms & Risk Factors
Extreme pain, deep red erythema, localized heat, and marked swelling typically peak within hours. It most commonly targets the first metatarsophalangeal (1st MTP) joint of the big toe. Modifiable risks include high BMI, high alcohol consumption, and purine-rich diets.
Diagnostic Investigations
The gold standard is microscopic joint aspiration showing needle-shaped, negatively birefringent uric acid crystals. Rapid, dramatic resolution following a therapeutic dose of colchicine also supports a presumptive clinical diagnosis.
Differentiating Crystal Arthropathies
- Gout — Monosodium urate crystal deposition causing soft tissue swelling and chronic joint erosions (classic “punched-out” lesions).
- Pseudo-Gout — Acute clinical presentation identical to gout, but joint aspiration shows no uric acid crystals.
- CPPD — Calcium pyrophosphate crystal deposition featuring rhomboid-shaped crystals and visible calcification of articular cartilage.

What is Polymyalgia Rheumatica (PMR)?
PMR is a primary care-managed autoimmune inflammatory condition characterized by widespread muscle pain and severe stiffness. Unlike arthritis, which targets the joint capsule, PMR targets the musculotendinous junctions leading to bilateral widespread girdle pain.
- Symptoms: Rapid onset of bilateral shoulder and pelvic/buttock pain, with profound morning stiffness lasting longer than 45 minutes.
- Demographics: Peak age of onset is 65 years, extremely rare under 50, with a 3:1 female-to-male ratio.

PMR Classification & Management
To differentiate PMR from mimics, patients must meet mandatory criteria: age 50+, bilateral shoulder pain, and abnormal ESR/CRP.
- Medical Management: PMR is treated primarily using oral corticosteroids (Prednisolone). True PMR shows rapid, dramatic resolution within 48 to 72 hours.
- Role of Physical Therapy: MSK therapy is strictly indicated to manage functional deficits, maintain joint range of motion, and preserve skeletal muscle bulk during long-term corticosteroid use.

Giant Cell Arteritis Emergency
GCA is a systemic inflammatory vasculitis affecting medium and large arteries. It is highly associated with PMR, and delayed treatment can cause irreversible, permanent ischemic blindness.
- Temporal Headache & Scalp Tenderness: Severe, unilateral headache for >48 hours and extreme pain when brushing hair or wearing a hat.
- Jaw Claudication & Visual Disturbances: Ischemic pain or cramping in the jaw muscles when chewing, along with transient unilateral vision loss or double vision. Refer immediately to the ED.
Frequently Asked Questions
Early acute flares typically self-resolve within a few days to two weeks. Because it is self-limiting, first-contact clinicians often do not evaluate patients until their second or third recurrence.
Giant Cell Arteritis (GCA) is highly associated with PMR. While PMR affects the musculotendinous junctions, GCA is a systemic inflammatory vasculitis affecting arteries. GCA is a medical emergency; delayed treatment can cause irreversible blindness.
Patients must first meet three mandatory entry criteria: Age 50+, bilateral shoulder pain, and abnormal ESR and/or CRP. A clinical score is then calculated based on morning stiffness (>45 mins), hip pain, absence of Rheumatoid Factor, and absence of other joint involvement.
While physical therapy does not resolve the underlying autoimmune inflammation, it is highly recommended to manage functional deficits, maintain joint range of motion, and preserve your skeletal muscle mass during long-term corticosteroid therapy.
